We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details. 

A Glimpse into your First Visit 
Please plan to arrive 10-15 minutes early to complete some necessary paperwork. 

Personal and medical history:
Your overall medical health has a lot of effects on your dental health, and vice versa. Please bring the name of your medical doctor to your first appointment. If you have had recent surgery, we will also need to know the name and telephone number of your surgeon. Also, bring a list of any prescription or over the counter medication that you may be taken.

Insurance Information:
Please bring your insurance card and your benefits book, if you have it.

Meeting with the Doctor
After completing the necessary paperwork, Polly will take you to one of our treatment rooms. She will make sure you are comfortable and ask you a few preliminary questions.
Dr. Hirsch will then come in, introduce himself, and spend a few minutes reviewing your medical history and listening to your wants and concerns related to your dental treatment. Dr. Hirsch will do a comprehensive medical history as it relates to your dental needs. Dr. Hirsch will then exit the room while Polly takes the necessary x-rays. 

When Dr. Hirsch returns to the room, he will complete a comprehensive dental examination including:

  • Hard and soft tissue cancer screening
  • TMJ evaluation
  • Periodontal evaluation
  • Diagnose the full mouth set of x-rays, after visual inspection, for cavities
  • Propose a treatment plan that is individualized to your dental needs.

After all your questions have been answered, Polly will take the time to answer any questions you may have about the financial aspect of the treatment. This usually involves providing you an estimate of your dental treatment, deciding if anything needs to be preauthorized to your insurance company, and settling on a payment option.
